In this role, you will support young people with learning disabilities as they transition from children’s services into adult services, helping them achieve positive outcomes while earning a competitive rate of £30 per hour.

What you will do:
Carry out Care Act assessments and reviews to identify the needs of young people transitioning into Adult Social Care.
Develop personalised Care and Support Plans that promote independence and positive outcomes.
Manage a statutory caseload while ensuring all documentation and processes are completed accurately.
Work closely with families, education teams, healthcare professionals, and partner agencies to provide holistic support.
Advocate for young people and guide them through the adult social care system.
