Key Responsibilities

• Develop tailored discharge plans to support individual recovery goals

• Work collaboratively with Acute Mental Health Units and community-based teams to ensure timely discharges

• Identify and support individuals at high risk of readmission with proactive intervention

• Provide structured follow-up within the community to maintain wellbeing and reduce relapse

• Collaborate with multidisciplinary partners including voluntary agencies, housing services, and substance misuse providers

• Engage effectively with families, carers, and community networks to encourage positive outcomes

• Uphold safeguarding responsibilities, promoting the safety and wellbeing of adults at risk

• Ensure appropriate access to local services and coordinated ongoing care
