We have an exciting opportunity for a skilled Band 6 Community Psychiatric Nurse (CPN) to join the Primary Care Liaison Service (PCLS) team within the Avon & Wiltshire Mental Health Partnership NHS Trust.

Based in the historic market town of Devizes, Wiltshire, this ongoing full-time role offers you the chance to make a tangible difference to patients’ lives while enjoying a competitive hourly rate of £26.78. What You Will Do

• Conduct comprehensive mental health assessments for adults referred by GPs and other primary care professionals.

• Develop and implement personalised care plans, applying recovery-focused and trauma-informed approaches.

• Deliver short-term clinical interventions, including medication monitoring, psychoeducation, and risk management.

• Collaborate effectively with GPs, IAPT services, community mental health teams, and secondary care services to ensure integrated care pathways.

• Participate in multidisciplinary meetings and case discussions, providing clinical insight and support.

• Carry out risk assessments and maintain up-to-date risk management plans.

• Ensure accurate and timely clinical documentation in line with Trust policy and NMC standards.

• Provide mentorship and supervision to junior staff, students, and new team members.

• Engage in service development and quality improvement projects, contributing to ongoing innovation within the team.
