We have an exciting opportunity for a skilled Band 6 Community Psychiatric Nurse (CPN) to join the Primary Care Liaison Service (PCLS) team within the Avon & Wiltshire Mental Health Partnership NHS Trust.
Based in the historic market town of Devizes, Wiltshire, this ongoing full-time role offers you the chance to make a tangible difference to patients’ lives while enjoying a competitive hourly rate of £26.78. What You Will Do

Conduct comprehensive mental health assessments for adults referred by GPs and other primary care professionals.

Develop and implement personalised care plans, applying recovery-focused and trauma-informed approaches.

Deliver short-term clinical interventions, including medication monitoring, psychoeducation, and risk management.

Collaborate effectively with GPs, IAPT services, community mental health teams, and secondary care services to ensure integrated care pathways.

Participate in multidisciplinary meetings and case discussions, providing clinical insight and support.

Carry out risk assessments and maintain up-to-date risk management plans.

Ensure accurate and timely clinical documentation in line with Trust policy and NMC standards.

Provide mentorship and supervision to junior staff, students, and new team members.

Engage in service development and quality improvement projects, contributing to ongoing innovation within the team.
