What you will do:

- Provide therapy to 5 patients in pathway 2 beds, delivering impactful in-patient rehabilitation services.
- Work collaboratively with a multidisciplinary team to ensure patient-centred care.
- Regularly assess and review patient progress, creating tailored rehabilitation plans.
- Offer guidance and support to patients and their families, helping them achieve the best possible outcomes.
- Maintain accurate and detailed records to facilitate efficient and effective care delivery.

Requirements:

- Valid HCPC registration.
- At least 2 years of professional experience in a similar role, ideally within the NHS or UK healthcare system.
- A car driver is preferable to assist with potential community visits and to offer greater service flexibility.
