What you will do:
- Coordinate care for complex cases under the Care Programme Approach (CPA)
- Develop co-produced care plans with service users
- Conduct comprehensive risk assessments and manage risks effectively
- Ensure safeguarding practices are upheld
- Perform holistic assessments of mental health and social care needs
- Conduct Care Act assessments and maintain regular reviews
- Engage with carers and wider support networks
- Foster partnership working with other statutory and voluntary organisations
- Liaise with inpatient units for a cohesive approach to care
- Keep accurate records and write detailed reports

Requirements:
- Registered with Social Work England
- Degree or equivalent in Social Work
- Experience working with adults with psychosis and complex needs
- Proficiency in Care Act assessments and reviews
- Knowledge of key legislation, including Safeguarding, Mental Health Act, Care Act, and Mental Capacity Act
