What you will do:

  • Work collaboratively within a multi-agency environment, sharing information and participating in joint assessments and care planning.
  • Promote and safeguard the welfare of vulnerable adults, ensuring their well-being is prioritized in all interactions.
  • Manage a diverse caseload, coordinating care and undertaking mental capacity assessments in accordance with the Mental Capacity Act 2005.
  • Implement the Care Act 2014, ensuring compliance in all aspects of your social work practice.
  • Engage with service users, carers, and fellow professionals to minimise and manage risks, empowering service users to maintain their preferred lifestyles.
  • Keep senior practitioners informed of any significant case developments while providing advice and consultancy within the team and to external stakeholders.
  • Participate actively in multi-disciplinary meetings, supervision, and training to improve personal and service development.
  • Adhere to health and safety policies, safeguarding protocols, and data protection legislation while carrying out your duties effectively.
