About the Role

Key Responsibilities

Deliver specialist diabetes management and treatment plans in community and clinic settings.

Support patients and families with education and guidance on lifestyle changes and self-management strategies.

Collaborate effectively with GPs, dietitians, podiatrists, and other healthcare professionals to coordinate care.

Conduct comprehensive assessments and tailor care plans to meet individual patient needs.

Monitor patient outcomes, adjusting care interventions and treatment as required.

Essential Requirements

Registered Nurse (Adult) with active NMC registration.

Proven experience in diabetes care within community or primary care settings.

Strong patient education and communication skills.

Ability to work autonomously while being part of a collaborative multidisciplinary team.

Excellent clinical judgment and commitment to patient-centred care.
Desirable Skills & Experience

Postgraduate training or certification in diabetes management.

Experience in insulin initiation and titration, blood glucose monitoring, and dietary support.
