What you will do

- Deliver high-quality, skilled nursing care to patients in their homes.
- Provide rapid response, rehabilitative, and post-acute care across community settings.
- Work shifts that span from 8 am to 10 pm, 7 days a week, including weekends and bank holidays.
- Utilize your specialist skills to holistically assess patient needs and develop short-term care programmes based on evidence-based practice.
- Focus on service priorities, such as admissions avoidance and timely supported discharge.
- Manage your caseload and support your team with the guidance of the Team Leader.
- Supplement locality-based planned care teams and collaborate with other health services when acute or intensive care is needed.
- Actively engage in service development activities to enhance care delivery.

Requirements::

- RGN qualification and NMC registration with a valid live pin.
- Recent UK nursing experience, particularly in Community or District Nursing.
