What you will do:
- Conduct comprehensive Care Act assessments, evaluate mental capacity, and engage in Mental Health Act duties.
- Develop and implement s117 discharge plans in collaboration with individuals and their families.
- Undertake regular reviews to ensure service user's needs are met and adjust care plans as necessary.
- Address safeguarding concerns promptly and effectively, prioritising the wellbeing of service users.
- Provide duty cover as part of the team to manage immediate risks and crises.
- Foster relationships with internal teams, including those focusing on learning disabilities, physical disabilities, and services for older adults.
Requirements of the Social Worker:
- A degree or equivalent qualification in social work.
- Registration with Social Work England.
- Significant frontline experience within UK Social Services as a qualified Social Worker is essential.
