Locum Social Worker for Adults Hospital Discharge Team in Halifax UK, £35 Hourly, Ongoing

Step into an invigorating role as a Locum Social Worker in our dynamic Adults Hospital Discharge Team, based in vibrant Halifax UK. With a generous hourly pay of £35 and the opportunity to make a tangible difference every day, this position covers both Huddersfield Royal Infirmary and Calderdale Royal Hospital. You will play a pivotal role in assisting service users as they transition from hospital care either back to their homes or into care home placements. The role is full-time, spanning 37 hours per week, and may also require assistance in our Post Discharge Assessment Team, ensuring the best ongoing community support for recently discharged individuals.

What you will do:

  • Conduct detailed hospital discharge assessments, ensuring safe and efficient transitions from the hospital to home or care facilities.
  • Collaborate with multi-disciplinary teams to provide holistic and person-centred care planning.
  • Review and monitor the support provided post-discharge, adjusting care plans to address evolving needs.
  • Provide guidance to and mentor Level 1 and 2 Social Workers and student trainees, fostering a culture of learning and development.
  • Ensure compliance with statutory duties, maintaining accurate records in line with organisational policies.
  • Contribute to the provision of day duty services, ensuring support is available to service users when they need it most.
