What you will do:

- Conduct comprehensive assessments of patients' social, emotional, and practical needs in line with hospital discharge protocols.
- Undertake ongoing assessments to determine appropriate care requirements, providing practical support and advice to patients and their families.
- Work closely with hospital teams to develop and implement person-centred discharge plans, ensuring a safe and efficient transition from hospital to community care.
- Follow safeguarding procedures to protect vulnerable adults from abuse and neglect while collaborating with safeguarding teams as needed.
- Collaborate with multi-disciplinary teams across the hospital and community to ensure the best outcomes for patients.
- Maintain accurate documentation and reporting, ensuring all case work is recorded on electronic health record systems.
