What you will do:

• Conduct comprehensive assessments of needs and provide expert advice and guidance to service users.
• Collaborate with contacts from different sectors, including health and voluntary sectors, to ensure effective communication and knowledge sharing.
• Review and monitor care plans, adjusting as needed to enhance quality and support independence through reablement programs.
• Manage your own caseloads or specific projects, ensuring specialist inputs are integrated and effective outcomes are achieved.
• Raise awareness of diverse care issues, carry out risk assessments, and develop safe methods of work practice guides.

Requirements of the Social Worker:

• Degree or equivalent in social work.
• Current Social Work England registration.
• Significant frontline experience in Social Services within the UK as a qualified Social Worker.
• A full clean driver’s licence and access to a car (or alternative means of travel) for visiting various locations, including home visits.
