What you will do:

- Undertake timely and effective physiotherapy assessments for patients referred to the Discharge to Assess service.
- Develop and implement evidence-based treatment and rehabilitation plans to promote patient wellbeing.
- Collaborate effectively with the multidisciplinary team to facilitate safe, efficient, and appropriate patient discharges.
- Set realistic and achievable patient-centred goals to enhance independence and recovery.
- Monitor, evaluate, and adapt treatment programs to meet patient needs and progress.
- Assess patients’ functional abilities, providing recommendations for mobility aids, equipment, or community services as required.
- Contribute to discharge planning through accurate documentation and effective communication with stakeholders.
- Support the coordination of care across health and social services to reduce the risk of hospital re-admissions.
- Offer professional advice and guidance to patients, families, and carers on rehabilitation and self-management strategies.
- Maintain accurate, up-to-date clinical records consistent with professional and service standards.

Requirements:

- HCPC registration as a Physiotherapist is required.
- Previous NHS experience at Band 6 level, ideally within discharge planning or acute/rehabilitation settings.
- Strong clinical reasoning, organisational, and communication skills.
- Ability to work effectively as part of a multidisciplinary team.
