What You Will Do:

• Assess and evaluate service users’ needs to determine appropriate aids and adaptations.

• Develop, review, and prescribe major adaptation plans to enhance the independence and quality of life for service users.

• Collaborate with a multidisciplinary team to implement adaptation strategies, ensuring a cohesive approach.

• Provide guidance and support to service users and their families about available adaptation options.

• Maintain detailed records and reports to ensure a high standard of service delivery.

An essential requirement for this role is HCPC registration, ensuring you meet professional and ethical standards in your practice.
