What you will do:
- Conduct medicines reconciliation to ensure accurate and safe patient medication records.
- Review patients and make clinically relevant interventions to optimise therapeutic outcomes.
- Complete the processing of discharges, ensuring all medication is appropriately prescribed and issued.
- Utilize electronic prescribing systems efficiently.
- Deliver high-quality care by collaborating closely with multidisciplinary teams on the wards.
Requirements of the Pharmacist:
- You must have a valid GPHC Registration.
- Prior experience working as a Pharmacist within a UK-based NHS Pharmacy setting.
- NVQ or equivalent Pharmacist qualification is necessary to apply for this exciting opportunity.
