Job Purpose:
The role is essential in contributing to the effective operational delivery of Adult Social Care Services. As you identify and assess individual needs, you will plan and monitor bespoke support plans, safeguarding and improving the quality of life for those in need.
What you will do:
- Effectively manage a complex and varied caseload with appropriate supervision, ensuring adherence to policies, procedures, guidance, and legislation.
- Communicate confidently in complex or high-risk situations, attending multidisciplinary forums and acting as a chair when required.
- Undertake direct work with service users, carers, and families, complying with interagency safeguarding adults’ policy and procedures.
- Conduct assessments and co-design individual support plans with other stakeholders, including health professionals, carers, and advocates.
- Lead safeguarding enquiries, ensuring timely completion and dissemination of outcomes.
- Maintain and update high-quality reports, ensuring data is gathered lawfully and securely.
- Undertake capacity and best interests assessments, holding a critical reflection to inform the decision-making process.
- Collaborate within an integrated team of allied health professionals ensuring alignment with the Council’s goals.
