What you will do:
- Assess the physical and mental well-being needs and collaborate with service users, carers, and team members to plan, implement, evaluate, and monitor comprehensive care plans.
- Implement care plans that address the mental, physical, spiritual, psychological, and social needs of service users, ensuring sensitivity to a wide range of individual differences.
- Administer medication and provide service users with clear information on its effects while monitoring for contraindications.
- Maintain updated risk assessments and care plans for service users attending the clinic to ensure up-to-date and accurate care.
- Provide support understanding the social and personal consequences of mental illness for service users and carers, ensuring compassionate and informed assistance.
- Encourage service users to pursue self-determination and freedom of choice, while adhering to legal, ethical, and professional guidelines.
- Operate within established policy, professional, and legal frameworks, maintaining knowledge of relevant acts, codes, and clinical policies.
- Ensure high standards of written communication and data entry on the e-PJS, adhering to Trust policies and professional conduct codes.
- Foster open communication and collaborative working relationships with colleagues, contributing effectively to the process of MDT working.
