The locality team is central to ongoing care management interventions and includes a wide range of tasks such as coordination of annual reviews, supporting extra care housing, and facilitating direct payments. What you will do:
- Conduct assessments under the Care Act 2014, focusing on supporting younger adults with physical disabilities and sensory impairments, as well as older people.
- Facilitate the transition of young people from Children’s Services to Adult Services, ensuring a smooth and supportive process.
- Work collaboratively within a multidisciplinary framework, developing close working relationships with partner agencies.
- Take on the role of Duty Senior on a rota basis, undertaking safeguarding work and Best Interest assessments.
- Provide supervisory support to both unqualified and qualified Care Managers, utilising your essential supervisory experience.
