What you will do:
As a Band 8a Pharmacist, your responsibilities will include:
- Clinically screening medication charts on the ward
- Screening discharge prescriptions and one-stop orders
- Completing medicines reconciliation and handling ward-based MI queries
- Engaging in MDT working to ensure optimal patient care
- Conducting some clinical checking and ward CD audits
- Ordering medicines and collaborating closely with Medicines Optimisation technicians
- Interpreting blood results in relation to medicines.
Qualifications:
To apply for this role, you must be a registered pharmacist with the General Pharmaceutical Council (GPhC). Preferably, you will have hospital experience, but this is not essential if you have been qualified and working for more than one year. An interest or experience in frailty, stroke, brain injuries, or rehab is desirable. Knowledge of polypharmacy and deprescribing is advantageous. Being a car driver would be beneficial.
