Your work will ensure these individuals live independently while helping to manage risks effectively. What you will do:

- Carry out assessments under the Care Act 2014 and assist in the transition of young people from Children's services to Adult services.
- Manage ongoing care interventions and coordinate annual reviews of support plans.
- Work collaboratively within a multidisciplinary framework to support individuals' independence.
- Develop strong relationships with partner agencies to ensure a cohesive approach to care.
- Oversee safeguarding duties and conduct Best Interest assessments as needed.
- Supervise both unqualified and qualified Care Managers, ensuring quality care delivery and support.
