What you will do:
- Clinically screen medication charts on the ward
- Screen discharge prescriptions and one-stop orders
- Complete medicines reconciliation
- Handle ward-based MI queries and work in MDT settings
- Conduct ward CD audits and order medicines
- Collaborate closely with Medicines Optimisation technicians
- Interpret blood results in relation to medicines
Requirements:
- Registered pharmacist with the GPhC, ideally with hospital experience, but not essential if qualified and have been working for over a year
- Interest or experience in frailty, stroke, brain injuries, or rehab is preferred
- Knowledge of polypharmacy and deprescribing is advantageous
- HCPC registration required
- Full driving licence is necessary for travelling between sites
