About the Role
An exciting opportunity has arisen for a Band 6 Care Coordinator to join the Adults Community Mental Health Team in Manchester. This ongoing locum role offers the flexibility of full-time or part-time hours and a competitive hourly rate of £26.00. You will play a crucial role in supporting adults with severe and enduring mental health conditions, working within a multidisciplinary team to deliver high-quality, person-centred care.
Key Responsibilities

Care Coordination: Act as the named care coordinator for a defined caseload, developing, implementing, and reviewing individualised care plans in partnership with service users and their families.

Mental Health Assessment & Risk Management: Undertake comprehensive mental health and risk assessments, contributing to the formulation of risk management and relapse prevention plans.

Clinical Interventions: Deliver a range of evidence-based interventions, including CBT-informed approaches, psychoeducation, medication monitoring, and crisis intervention.

Multi-Agency Working: Liaise with a broad range of services including primary care, social services, housing, employment, and voluntary sector agencies to provide integrated and holistic care.

Safeguarding & Legislation: Apply safeguarding protocols and relevant legislation, including the Mental Health Act and Mental Capacity Act, in practice.

Record Keeping: Maintain accurate clinical records using systems such as PARIS or EMIS, ensuring compliance with GDPR and professional documentation standards.

Team Contribution: Engage in regular clinical supervision, reflective practice, and multidisciplinary team meetings. Contribute to service development and quality improvement initiatives.
Essential Criteria

Qualified Mental Health Professional (Registered Nurse, Social Worker, or Occupational Therapist).

Registration with the relevant UK regulatory body (NMC, HCPC, or SWE).

Experience in community mental health settings, preferably in a care coordination role.

Strong assessment, planning, and intervention skills with a recovery-focused approach.

Excellent organisational and communication abilities.

Familiarity with electronic clinical systems such as PARIS or EMIS is desirable.
