About the Role

An exciting opportunity has arisen for a Band 6 Care Coordinator to join the Adults Community Mental Health Team in Manchester. This ongoing locum role offers the flexibility of full-time or part-time hours and a competitive hourly rate of £26.00. You will play a crucial role in supporting adults with severe and enduring mental health conditions, working within a multidisciplinary team to deliver high-quality, person-centred care.

Key Responsibilities

• Care Coordination: Act as the named care coordinator for a defined caseload, developing, implementing, and reviewing individualised care plans in partnership with service users and their families.

• Mental Health Assessment & Risk Management: Undertake comprehensive mental health and risk assessments, contributing to the formulation of risk management and relapse prevention plans.

• Clinical Interventions: Deliver a range of evidence-based interventions, including CBT-informed approaches, psychoeducation, medication monitoring, and crisis intervention.

• Multi-Agency Working: Liaise with a broad range of services including primary care, social services, housing, employment, and voluntary sector agencies to provide integrated and holistic care.

• Safeguarding & Legislation: Apply safeguarding protocols and relevant legislation, including the Mental Health Act and Mental Capacity Act, in practice.

• Record Keeping: Maintain accurate clinical records using systems such as PARIS or EMIS, ensuring compliance with GDPR and professional documentation standards.

• Team Contribution: Engage in regular clinical supervision, reflective practice, and multidisciplinary team meetings. Contribute to service development and quality improvement initiatives.

Essential Criteria

• Qualified Mental Health Professional (Registered Nurse, Social Worker, or Occupational Therapist).

• Registration with the relevant UK regulatory body (NMC, HCPC, or SWE).

• Experience in community mental health settings, preferably in a care coordination role.

• Strong assessment, planning, and intervention skills with a recovery-focused approach.

• Excellent organisational and communication abilities.

• Familiarity with electronic clinical systems such as PARIS or EMIS is desirable.
