What you will do:
- Manage a caseload of complex mental health clients within the older adult service.
- Provide integrated health and social care interventions, commissioning personalised care packages.
- Conduct safeguarding investigations and continuously monitor clients' mental states for signs of relapse and pathways to recovery.
- Be an integral part of the Central AMHP team rota, undertaking assessments and coordination under the Mental Health Act.
- Collaborate with multidisciplinary teams to ensure comprehensive care delivery.
