What you will do:

- Conduct assessments of need and risk under the Care Act 2014 and other relevant legislation.
- Develop, implement, and review tailored care and support plans in partnership with service users, families, and professionals.
- Undertake safeguarding enquiries under Section 42 of the Care Act to ensure the safety and well-being of service users.
- Work within the framework of the Mental Capacity Act 2005 to make informed Best Interest decisions.
- Provide support to service users with complex mental health needs, including those under the Care Programme Approach (CPA).
- Collaborate effectively with NHS mental health services, community organisations, and housing providers to deliver holistic and integrated care.
- Offer evidence-based interventions and strength-based approaches to aid recovery and promote social inclusion.
- Maintain precise and timely case records using our electronic recording system.
- Participate in duty and triage systems to provide immediate support as required.
- Attend and contribute to multi-agency meetings, reviews, and Mental Health Tribunals.
- Engage in supervision, Continuing Professional Development (CPD), and reflective practice in alignment with Social Work England professional standards.
