Key Responsibilities:

  • Conduct holistic functional assessments focusing on physical, cognitive, and environmental needs.

  • Collaborate with individuals to develop personalised reablement goals and action plans (e.g., meal prep, dressing, mobility).

  • Recommend and facilitate the use of minor equipment and adaptations to aid independence at home.

  • Contribute to safeguarding assessments and actions in accordance with the Care Act and council protocols.

  • Maintain detailed, accurate case notes using Slough Borough Council’s digital case management systems.

  • Participate in team meetings, care reviews, and planning sessions to ensure effective care coordination.


Requirements:

Qualifications:

  • Degree or Diploma in Occupational Therapy

  • HCPC Registration (Health and Care Professions Council)

Experience:

  • Previous experience in adult social care, community reablement, or hospital discharge pathways is highly desirable.

Skills:

  • Strong understanding of the Care Act and manual handling practices

  • Able to prioritise caseloads and deliver outcome-focused interventions under time pressure
