What you will do:
- Conduct thorough assessments and discharge planning for adults on mental health wards to ensure a safe transition back into the community.
- Perform Care Act assessments, mental capacity assessments, and risk evaluations while participating in best-interest meetings.
- Write social circumstances reports and prepare for tribunal attendance.
- Actively participate in multi-disciplinary team meetings, including ward rounds, bed-flow discussions, and capacity planning sessions.
- Coordinate with a range of stakeholders, such as nursing staff, medical teams, occupational therapists, and local authorities, to arrange housing, care packages, and support services.
- Follow up with discharged patients, providing 7-day post-discharge check-ins and 6-week assessments before handing over to community teams.
- Engage in safeguarding practices by applying your expertise in relevant legislations like the Care Act and the Mental Health Act.
