What You Will Do:

• Conduct comprehensive assessments and discharge planning for patients with complex needs.

• Lead on the Discharge to Assess (D2A) process with autonomy and confidence.

• Work within the inpatient ward environment, delivering holistic and responsive care.

• Maintain accurate records using RIO – previous experience with the system is highly desirable.

• Undertake access visits regularly – car driver essential for travel requirements.

• Collaborate effectively within multidisciplinary teams to achieve best outcomes for patients.

Requirements:

• HCPC Registration – essential

• Experience in general medicine and frailty preferred

• Proficiency with RIO and discharge planning

• Must be a car driver due to the need for access visits
