Key Responsibilities:

• Complete holistic assessments and implement tailored physiotherapy treatment plans for frail and complex elderly patients.

• Lead on discharge planning, coordinating patient flow safely and effectively from hospital to home.

• Utilise the D2A (Discharge to Assess) model to optimise rehabilitation outcomes and support patient independence.

• Accurately document all assessments and interventions using RIO.

• Undertake access visits and community-based reviews as required – therefore, a car driver is essential.

Requirements:

• HCPC registration as a Physiotherapist is mandatory.

• Prior experience in inpatient rehab, discharge planning, and working with elderly populations.

• Proficiency with RIO and the D2A framework preferred.

• A valid UK driving licence and access to a vehicle are essential for access visits.
