What you will do:

- Conduct comprehensive needs assessments for individuals requiring hospital-based social work support.
- Develop, implement, and review care plans ensuring alignment with the Care Programme Approach.
- Deliver social work interventions that are tailored to the needs of service users and their carers.
- Coordinate with the multidisciplinary team to ensure a holistic approach to care and support.
- Monitor, review and evaluate the effectiveness of interventions to ensure continual improvement in service delivery.
- Provide expert advice and guidance on the Care Management process to your team and agency partners.
- Champion social inclusion and facilitate opportunities for service users to lead independent lives.
- Engage in safeguarding adults practises to protect those at risk, ensuring best outcomes for vulnerable individuals.
