What you will do:
- Receive and coordinate complex referrals and undertake resulting assessments, including those for Approved Mental Health Practitioner (AMHP) and Best Interest Assessor (BIA) and carers.
- Coach and mentor Social Care Assessors in their assessment, support planning, and review tasks.
- Provide leadership on risk and safeguarding matters, promoting personalised care pathways with an emphasis on choice and control for individuals.
- Work collaboratively to influence and shape the local social environment, promoting inclusion and accessibility, and feeding identified needs into strategic commissioning.
- Cover for your line manager and fellow social workers from other teams as required, demonstrating flexibility and adaptability.
