Social Worker - Leaving Care Team (16 to 18 years, children and young people) Location: Constantine House, Ipswich, Suffolk (Hybrid working available) Salary: £36.33 hourly, this is an ongoing, full-time locum opportunity.

What you will do:

- Manage a caseload of children and young people aged 16 to 18 within the care system.
- Conduct statutory visits as required, ensuring a minimum of monthly engagement to monitor and support their Care Plans.
- Prepare child's care and pathway plans, facilitate their reviews, and ensure compliance with statutory requirements.
- Participate in a duty rota that supports team consistency and response to needs.
- Assist young people in their preparation for independence, offering guidance and support tailored to their unique situations.
- Attend and contribute to professional meetings, liaising with multiagency professionals to foster comprehensive, wrap-around support.
- Collaborate closely with partner agencies to ensure a cohesive approach to each young person’s care.
