What you will do:

1. Conduct holistic assessments of the young person's needs, covering all areas including physical, emotional, educational, and social aspects.
2. Prepare and regularly review Pathway Plans, involving the young person, their family, and professionals for a coordinated approach.
3. Serve as the Personal Adviser for care leavers aged 18 and over, supporting them as they implement their Pathway Plan.
4. Support young people in securing suitable accommodation and assist with practical tasks like managing finances.
5. Provide essential advice on life skills necessary for independent living, covering vital topics like budgeting, employment, and health management.
6. Offer compassionate emotional and advocacy support, working with housing providers, educational institutions, and employers.
7. Collaborate with other professionals and agencies, maintaining a focus on holistic support.
8. Monitor and respond to changes in the young person's health needs, supporting them in managing risks and maintaining safety.
9. Manage a caseload effectively, engaging in reflective supervision and maintaining accurate records.
10. Practice ethically, ensuring confidentiality and facilitating smooth transitions between services when specialized knowledge is needed.
