Locum Social Worker - Adults with Learning Disabilities Team - Lewisham, UK - £32 Hourly - Contract: 3 Months Full Time

A thrilling opportunity beckons for a Locum Social Worker to join our Adults with Learning Disabilities Team in Lewisham. What you will do:

  • Provide timely and proportionate Care Act assessments for service users and their carers, identifying the outcomes they wish to achieve.
  • Empower service user independence and recovery, promoting Enablement and linking to community-based services for enhanced personal wellbeing.
  • Collaborate within multi-agency teams to deliver seamless health and social care services.
  • Involve service users and carers in individualised and creative support planning, promoting choice and control through personal budgets and Direct Payments.
  • Partner with Community Development Staff to aid access to community-based services.
  • Adhere to risk management and adult safeguarding procedures, remaining vigilant for children’s safeguarding concerns and liaising with Children’s Social Care Services as needed.
  • Conduct assessments and service user reviews to high standards in accordance with procedures, reflecting the needs and outcomes of service users and their carers.
