What you will do:
- Manage a defined caseload of clients with complex mental health needs, ensuring a person-centred and holistic approach.
- Conduct comprehensive assessments and design personalised care plans in accordance with the Care Programme Approach (CPA).
- Deliver evidence-based interventions tailored to promote recovery-focused outcomes.
- Act as a key coordinator between the multidisciplinary team and external agencies, ensuring seamless communication and continuity of care.

Requirements of the Community Psychiatric Nurse:
- Hold a degree or equivalent qualification in Mental Health Nursing.
- Have substantial experience working as a Mental Health Nurse.
- Current NMC Registration is required to apply for this position, ensuring you meet professional standards.
