What you will do:

• Complete Care Act assessments and reviews within an integrated team setting.
• Conduct Mental Capacity Assessments (MCA) and Strengths-Based Framework (SFG) evaluations.
• Participate in safeguarding activities and assessments for Continuing Healthcare (CHC).
• Develop strong professional relationships within the team and maintain curiosity and challenge within your practice.
