Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Qualified Social Worker within the Initial Contact Team to work Full Time based in Swindon.

The salary for this Qualified Social Worker job is up to £43,693 per annum.

Main duties:

• You will join us in Adult Social Care in a friendly, supportive and dynamic team, bringing a particular interest in and focus on Adult Safeguarding.
• This role will see you working closely with the residents of Swindon and their families as well as a number of professionals including health, provider services, and safeguarding and mental health colleagues.
• The Adult Social Care Teams support adults living in Swindon and you will be responsible for supporting people contacting Adult Social Care and completing assessments and proportionate support planning. You will follow the care management process of assessment, reviews, and completing Mental Capacity Assessments and Best Interest decisions.
• To provide statutory support to adults with care and support needs and their carers/families, which includes information and advice, assessment, support planning and safeguarding.
• Work in partnership and liaison with partners including primary, secondary and tertiary health care, private providers, voluntary sector organisations and others to provide coordinated support for individuals and/or carers.
• To undertake person-centred social work with individuals and to plan for the provision of support and/or services with them in order to assist them to meet their outcomes and to live as independently as possible.
• To ensure accurate and timely recording of all information and activity onto the social care records database in accordance with work targets set by the Team Manager and Supervisor.
• To have a well-developed understanding of assessment, support planning and the management of risks for individuals and be able to apply Council policy and support people to manage risk appropriately.
• To contribute proactively to the effective working of the team with positive attitude, by sharing knowledge, offering advice and support and by preparedness to be involved in the development of services.
• To fully engage and prepare for own supervision and appraisal through self-evaluation, ensuring that continual professional development is evidenced.

Requirements of the Qualified Social Worker job:

• A social Work Qualification or equivalent.
• Understanding of relevant legislation.
• Registered or eligible for registration.
