Qualified Social Worker – Adults Safeguarding Team within Rushcliffe, Nottinghamshire UK, earning £26.51 hourly on a locum basis for 3 months. What you will do:
- Manage a complex caseload and be responsible for the assessment, support planning, and review of complex individual needs, ensuring outcomes promote independence.
- Carry out comprehensive assessments using a range of health and social care tools and contribute to continuing healthcare assessments.
- Identify and facilitate access to reablement and enablement services to enhance individual wellbeing and independence.
- Champion a Strength Based Approach, ensuring individuals can assess their needs and risks to achieve their short- and long-term goals.
- Collaborate with community resources and natural support networks, leveraging individual assets and preventative services.
- Monitor and review ongoing service provision, coordinating with all parties to focus on outcomes and wellbeing.
- Liaise and negotiate with local providers and support networks to deliver improved outcomes tailored to individuals.
- Provide detailed information, advice, and support to individuals, their carers, staff, and colleagues alike.
- Undertake safeguarding work in the capacity of a Safeguarding Officer.
