Job Description: Band 6 Occupational Therapist - Discharge to Assess Team - Basildon, UK - Earn up to £26 per Hour. Full-Time, Ongoing.

Are you a highly motivated and enthusiastic Band 6 Senior Occupational Therapist looking for an exciting opportunity to make a tangible difference? Join our vibrant Discharge to Assess Service team in Basildon, where you will collaborate with Senior Physiotherapists and CHC/AACC Nurses along with domiciliary care staff and care home staff. Together, you'll be delivering exceptional service in our Clinical Support Service division. This full-time and ongoing position gives you the chance to thrive while honing your skills, all while earning up to £26 per hour.

What you will do:

  • Conduct comprehensive assessments of patients to create effective care and discharge plans.
  • Work closely with a multidisciplinary team to ensure seamless patient transitions from hospital to home or other care settings.
  • Liaise with families and carers to support the discharge process and provide necessary education and advice.
  • Monitor and evaluate patient progress, adjusting plans as needed to optimise outcomes.
  • Maintain accurate records and documentation in line with organisational policies and professional standards.


As a vital member of our team, you will lead by example and contribute significantly to our mission of delivering excellent patient care. A current HCPC registration is essential, and given the nature of the job, a valid driving licence is required to visit patients off-site.
