What you will do:

  • Facilitate mental health assessments and provide early treatments within Primary Care, ensuring smooth transitions for patients between primary and secondary services.
  • Collaborate with community services to weave a robust network of support for mental health patients, fostering relationships that ease patient access and care.
  • Coordinate care plans, screen, assess, and provide brief productive interventions, helping individuals set and achieve their health goals.
  • Support and guide the Community Connections Primary Care Link Worker, ensuring comprehensive care that ties physical health considerations into mental health treatment.
  • Develop training modules to upskill the Primary Care Team in the management of mental and physical health integration, bolstering service quality.


Qualifications required:

  • Professional Qualification in Mental Health Nursing, Occupational Therapy, or Social Work.


Experience required:

  • NHS experience and familiarity with SystemOne and EMIS.
  • Experience with evidence-based mental health interventions for various conditions.
