What you will do:

• Provide information and advice regarding service eligibility under Self Directed Support legislation.
• Conduct basic and comprehensive assessments of needs in line with Council Service Standards.
• Perform risk assessments and monitor Adult Support and Protection Plans.
• Collaboratively create care plans with service users and carers.
• Negotiate with internal and external care providers to develop effective care packages.
• Maintain accurate and confidential records in accordance with data protection legislation.
• Engage in team activities and training, ensuring active participation in team meetings and development sessions.
• Work within the Adults Duty system and represent Social Work Services in various forums.
