What you will do:

  • Ensure service users and their carers receive timely and proportionate care management assessments, identifying desired outcomes.
  • Promote user independence and recovery through Enablement and link them to community services that enhance wellbeing.
  • Collaborate as part of multi-agency teams providing seamless health and social care services.
  • Partner with Support Planning Officers to engage users and carers in individualized support planning, using personal budgets including Direct Payments as the default.
  • Work with Community Development Staff to connect people with community services.
  • Adhere to risk management and adult safeguarding procedures in practice, being vigilant about potential children’s safeguarding needs and liaising with Children’s Social Care Services.
  • Conduct assessments and reviews to a high standard and in a timely fashion, reflecting the needs and outcomes of service users and carers.
  • Collaborate with advocates and carers to achieve outcomes for users who lack mental capacity, in line with the Mental Capacity Act.
  • Ensure accurate and timely casework recording on electronic systems, maintaining service user and carer information.
  • Manage own workload and seek supervision for advice and reflective practice.
  • Base decision making on research and evidence, consulting with others.
  • Prepare required reports and promote joint working with colleagues across departments and health services.
  • Maintain safety awareness for self and others, alerting management to high-risk situations.
  • Adhere to all relevant legislation and policy, ensuring Equal Opportunities and Environmental Policies in service delivery.
  • Maintain HCPC registration and follow the Council’s Code of Conduct.
