What you will do:
- Deliver the Healthy Child Programme (0–5) through universal and targeted visits, tailoring your approach to meet the diverse needs of the community.
- Conduct comprehensive assessments to identify health needs and risks for children and families.
- Provide expert support on a range of topics, including infant feeding, child development, immunisations, and maternal mental health, ensuring families are informed and supported every step of the way.
- Recognise and address safeguarding concerns, attending relevant meetings and adhering to child protection procedures.
- Develop and implement personalised care plans in collaboration with families and other professionals, ensuring holistic care delivery.
- Collaborate with GPs, midwives, social workers, and early years services to provide cohesive support networks.
- Maintain accurate and timely electronic health records, ensuring information integrity and accessibility.
- Promote health education and actively participate in public health initiatives to support community well-being.
- Provide guidance and mentorship to students and junior staff when needed, fostering an environment of growth and learning.
- Engage in continuous professional development and actively participate in team activities and supervision.
