What you will do:
- Prepare for social work contact, including reviewing case notes and gathering pertinent information.
- Conduct thorough assessments of service users, carers, and families, especially in challenging circumstances, and assist in developing comprehensive Care and Support Plans.
- Create, implement, and update Care and Support Plans, ensuring the well-being and health needs of those you support.
- Empower service users, carers, and families to make informed decisions through Self Directed Care and Individual Budgets, optimising resource use.
- Be vigilant and proactive concerning safeguarding issues, adhering to appropriate policies.
- Assess and manage risks, working towards positive behaviour changes to minimise risk.
- Respond swiftly to crisis situations, using legal frameworks where necessary.
- Ensure high-quality, accurate data entry on social care systems and maintain confidentiality.
- Participate in professional meetings, advocating effectively for service users and families.
- Build strong partnerships with agencies in health and the voluntary sector.
Requirements:
- Demonstrated experience in conducting assessments and developing Care and Support Plans.
- Ability to work independently, managing priorities and maintaining detailed records.
- Experience in a multidisciplinary team, providing holistic social work services.
- Thorough understanding of relevant legislation and best practices in the field.
