As part of Kingston's Learning Disability Team, based at Sessions House in Surbiton, you'll adopt a hybrid working style. This flexible approach allows you to work remotely, attend necessary visits, and engage in monthly face-to-face team meetings. The team comprises a wealth of experience, skill, and dedication, including a Team Manager, two Senior Social Workers, and support coordinators. You'll also collaborate with a Transition Team led by a Senior Transition Coordinator.

What you will do:

- Conduct Care Act assessments for people in supported living and residential care placements
- Engage and work closely with individuals experiencing transitions
- Undertake reviews and participate in safeguarding procedures
- Utilise your effective communication and IT skills to collaborate with a diverse range of professionals and agencies
- Form effective working relationships and contribute to a stable, knowledgeable team environment
