What you will do:

• Serve as the primary contact point for referrals into Adult Social Care
• Screen and assess referrals, ensuring appropriate and timely responses
• Signpost to relevant services and provide Information, Advice, and Assistance
• Work collaboratively with the Early Response team to coordinate service delivery
• Maintain accurate records and documentation in line with organisational requirements
• Engage with a range of stakeholders, building effective relationships to enhance service provision
