What you will do:

• Manage a diverse caseload with the support of the Deputy Team Manager, ensuring each child has an individualised care plan.
• Assess ongoing needs and risks, ensuring children and young people receive the best possible care and progression.
• Work closely with care leavers, assisting in the development of pathway plans to support them through to early adulthood.
• Implement statutory duties under Part 6 of the Social Services and Wellbeing Act.
• Collaborate with the leaving care service to ensure seamless and consistent support for young people.
