Role Responsibilities Deliver specialist assessment and intervention for stroke and non-stroke acute pathways. Work collaboratively with the multidisciplinary team to create and implement individualised, evidence-based care plans. Provide education, coaching and support to patients, families and carers to enhance communication and rehabilitation outcomes. Participate in departmental meetings and contribute to service development and quality improvements. Maintain accurate, up-to-date patient documentation, adhering to HCPC professional standards. Requirements HCPC-registered Speech and Language Therapist. Minimum of 2 years’ experience in stroke or acute care settings.
