What you will do:

- Provide one-to-one and group support to young people and families affected by substance misuse.
- Offer practical advice on harm reduction, healthy coping mechanisms, and life skills development.
- Conduct assessments to understand substance misuse and related issues.
- Develop and implement individual care plans with clear goals and regular reviews.
- Deliver substance misuse education and recovery workshops to young people and families.
- Support families in understanding the impact of substance misuse and accessing necessary treatment.
- Ensure the safety and wellbeing of young people through safeguarding procedures.
- Identify and address any risks to client safety or wellbeing, including signs of relapse or mental health concerns.
- Work with multi-agency teams (social workers, healthcare providers, schools) to deliver holistic support.
- Participate in case meetings to ensure coordinated, effective care for young people and families.
- Maintain accurate, up-to-date records of assessments, care plans, and client progress.
- Provide regular reports to senior staff and external agencies as required.
