What you will do:

• Lead and manage a team of social workers and support staff within the Localities Team to deliver exceptional community-based services.
• Collaborate with the Community Response and Reablement Team to provide crucial information, advice, and services, including occupational therapy and reablement for new community members.
• Work closely with the Community Independence and Wellbeing Team, supporting existing individuals with community social work and occupational therapy to enhance their wellbeing.
• Oversee the Hospital Team and ensure 7-day a week social work and social care support for seamless hospital discharges.
• Engage with community teams to support individuals with learning disabilities and mental health needs, ensuring they receive tailored care and support.
