What you will do

Manage a caseload of adult service users with substance use issues, including alcohol, non-opiates, and opiates, ensuring structured and personalised recovery plans are in place.

Deliver evidence-based interventions tailored to individual recovery goals and plans.

Provide specialist substance misuse interventions and practical support to promote harm minimisation and safer substance use.

Collaborate with service users to identify and achieve personalised recovery goals, enhancing their skills and confidence to sustain recovery.

Liaise closely with GPs, social care professionals, health services, and other key partners to coordinate comprehensive care.

Advocate for positive health and wellbeing outcomes for all service users, contributing to a supportive and recovery-focused environment.

Work from the main office in Forest Hill and within GP Shared Care settings across Lewisham.
